Patient Forms, Map and Directions to Gurnee Office

We would like to welcome you to our office and give you a brief over view of what we so proudly provide during your initial exam.  Your initial exam will include:

  • A review of your medical and dental history
  • X-rays as indicated
  • Photos of your teeth (reviewed with you as both an educational tool and record keeping.)
  • An oral cancer screening.
  • Documenting existing restorations and missing teeth
  • Identifying decay, cracks in teeth and failing restorations.
  • An occlusal examination (your bite and how it affects your teeth).
  • A periodontic examination (to determine the health of your gums and supporting bone).
  • TMJ examination (the health of your jaw joints)

Please note: a dental cleaning is NOT provided during the initial exam.

Upon completion of your initial exam, Dr. Mackey will review our findings with the aid of your dental x-rays, your photos, and any possible study models acquired.
